Abstract

Rantai pasok cacing tanah hidup pada CV Mega Surya Nusantara memiliki berbagai risiko yang dapat menghambat kelancaran aktivitas mulai dari perencanaan hingga pengiriman produk kepada pelanggan. Penelitian ini bertujuan mengidentifikasi dan menganalisis risiko serta menentukan tindakan mitigasi yang menjadi prioritas dalam pengelolaan risiko rantai pasok. Pemetaan aktivitas dilakukan menggunakan model Supply Chain Operations Reference (SCOR), sedangkan analisis risiko menggunakan metode House of Risk (HOR) Fase 1 dan Fase 2. Hasil pemetaan menunjukkan bahwa rantai pasok terdiri atas 5 proses utama, yaitu Plan, Source, Make, Deliver, dan Return, dengan total 21 aktivitas. Hasil identifikasi dan validasi menghasilkan 21 risk event dan 16 risk agent. Berdasarkan perhitungan Aggregate Risk Potential (ARP) dan analisis Pareto pada HOR Fase 1, diperoleh 10 risk agent prioritas yang menjadi dasar penentuan tindakan mitigasi. HOR Fase 2 menghasilkan 10 preventive action, dengan PA2 berupa standardisasi SOP budidaya dan sosialisasi kepada pekerja sebagai prioritas pertama dengan nilai Effectiveness to Difficulty (ETDk) sebesar 21.925, diikuti PA7 sebesar 20.019, PA4 sebesar 17.948, PA3 sebesar 16.823, dan PA6 sebesar 15.373. Hasil penelitian menunjukkan bahwa perusahaan perlu memprioritaskan tindakan mitigasi berdasarkan efektivitas dan tingkat kesulitan penerapannya serta menerapkannya secara bertahap sesuai kondisi operasional perusahaan.
